    Oncogenoma Precision Oncology

    Ipilimumab is a monoclonal antibody belonging to the class of immune checkpoint inhibitors, used in the treatment of various malignancies, particularly metastatic or unresectable melanoma. The introduction of this drug represented one of the major innovations in cancer immunotherapy, improving the survival of many patients.

    Ipilimumab works by blocking the protein CTLA-4 (Cytotoxic T-Lymphocyte Antigen-4), a checkpoint molecule present on T lymphocytes that plays a role in limiting the immune response.

    Under physiological conditions, CTLA-4 protects the body from excessive immune system activation. However, tumor cells can exploit this mechanism to evade immune defenses.

    By blocking CTLA-4, Ipilimumab promotes greater activation of T lymphocytes, enhancing the immune system's ability to recognize and fight tumor cells.

    Ipilimumab in the treatment of metastatic melanoma

    Clinical studies published in the New England Journal of Medicine (NEJM) and presented at the American Society of Clinical Oncology (ASCO) have demonstrated that ipilimumab can improve survival in patients with advanced metastatic melanoma, marking a major shift in the therapeutic approach to this disease.

    The research also highlighted that the combination with the experimental gp100 vaccine did not determine a significant clinical benefit compared to the use of ipilimumab alone.

    Immune system activity

    Even in brain metastases, although Ipilimumab does not directly cross the blood-brain barrier, the drug stimulates T lymphocytes, which can also reach brain metastases from melanoma.

    In some cases, the reintroduction of Ipilimumab after a previous therapeutic response has allowed for new disease control, while combination protocols with other immunotherapies, such as interleukin-2 (IL-2), have also been studied.
